Step 1: Use the Request Appointment button to schedule an intake session and complete pre-screener questions. IMPORTANT: For minor clients, both parents are required to attend the intake session.

Step 2: You will receive an email with a link to complete the intake paperwork and sign consent forms. Please return this as soon as possible, as your intake session cannot happen without it.
